You can submit a partnership termination request with two methods: 
A. Method 1. Submit a partnership termination request email without signature:
1. Fill in the partnership termination request form here https://gjk.id/gocorppengakhirankerjasama
2. Submit the completed form in PDF file to the Sales/Customer Experience Team through gocorp-cx@gojek.com.
3. Please ensure to submit the form through the Company’s Representative from Business or Finance as specified in the signed Commercial Terms.

Use the following partnership termination request template below:
Subject: GoCorp Partnership Termination Request - <Business/Company Name>

To Sales/Customer Experience Team GoCorp,
I’m <Your Name> from <Company Name>. I would like to request for a partnership termination for <Company Name>. Here I have attached the unsigned form for the request. Your assistance in this matter would be greatly appreciated.

B. Method 2. Submit a partnership termination request with signature.

2. Attach wet signature/e-signature

Who are the authorized parties to sign the partnership termination form? (Point B)
