How to claim Safe Trip+

When you submit a claim, please collect all evidence of the incident and required documents such as booking number, photo of identity card (KTP), medical resume, receipt or proof of original hospital fees, accident report from the Police or Police Official Report (BAP), and other documents are submitted in the form of a soft copy (photo) and sent via an online form at the time of filing a claim.

 

You can submit a claim on MyInsurance Page (only available for Android users who have Gojek app version 4.76 and above) or you can submit a claim via PasarPolis claim portal

After you fill out the claim form on the website, we or the insurance provider will contact you to ask for answers to several questions related to the events experienced to ensure the completeness of the information (such as what, who, why, when, where and how the chronology of events).

What documents are required for filing a claim?

Below are the complete documents or supporting evidence for each incident that you need to prepare before submitting an insurance claim:

 Death due to Accident:

  • Customer ID/KTP
  • Death certificate from local authorities
  • Heir's ID/ KTP
  • Family Certificate
  • Police Official Report (BAP) if the incident is caused by a traffic accident
  • Death certificate from hospital (if died in hospital)
  • Supporting medical documents - X-Ray, MRI, Lab Results (if any)

Permanent Disability:

  • Customer ID/KTP
  • Medical Resume
  • Police Official Report (BAP) if the incident is caused by a traffic accident
  • Supporting medical documents - X-Ray, MRI, Lab Results (if any)

Medical Expenses:

  • Customer ID/KTP
  • Medical Resume
  • Details of costs/ invoice for treatment at hospitals/ traditional treatment
  • Supporting medical documents - X-Ray, MRI, Lab Results (if any)
  • Certification of Traditional Treatment

Loss of Personal Items due to Theft/ Robbery:

  • Customer ID/KTP
  • Police Report
  • Photo evidence of physical violence

Hospital Cash Plan:

  • Customer ID/KTP
  • Medical Resume
  • Discharge letter from hospital stating number of hospitalization days

Is there a time limit for submitting a claim?

The submission of a SafeTrip+ claim related to an accident must be reported a maximum of 30 (calendar) days after the incident. You have to complete the claim documents required maximum of 45 (forty-five) calendar days after the first claim submission.
